Tongkat Ali for libido is one of the herb's oldest traditional uses and one of its most aggressively marketed modern claims. The human research is more interesting than a simple yes or no: some trials report improvements in desire, erection-related questionnaires or sexual wellbeing, while others are small, use combination products or study men with specific ageing-related symptoms.

To understand what the evidence means, libido must first be separated from testosterone, erection quality and fertility. They can influence one another, but they are not the same outcome.

Libido, erections and testosterone are different

What do human studies say about Tongkat Ali and libido?

The 109-man, 12-week trial

The most directly relevant study enrolled 109 men aged 30 to 55. Participants took 300 mg of a standardised water extract or placebo for 12 weeks. The Tongkat Ali group showed higher scores in the overall erectile-function domain and a reported 14% improvement in sexual libido by week 12. Sperm motility and semen volume also changed in selected analyses, while safety measures were comparable with placebo (Ismail et al., 2012).

This is a useful positive trial because it was randomised and placebo-controlled. It still tested one proprietary extract, and the percentage does not mean every man became 14% more interested in sex. Questionnaire outcomes describe group averages.

The small combination study

A separate 12-week trial tested a 300 mg combination of Tongkat Ali extract and Polygonum minus in men aged 40 to 65. Only 26 participants completed the study. The active group improved on several sexual-performance and erection-hardness measures compared with placebo (Udani et al., 2014).

Because this was a combination product and the completed sample was very small, it cannot isolate Tongkat Ali's effect. It supports interest, not a precise promise.

The six-month ageing-male trial

Forty-five men with androgen-deficiency symptoms were allocated to placebo, Tongkat Ali, exercise training or their combination for six months. Two hundred milligrams daily plus concurrent training improved erectile-function scores and total testosterone. The combined intervention makes it difficult to separate supplement and exercise effects (Leitao et al., 2021).

The wider evidence

A 2017 systematic review examined 11 human clinical studies relating to male sexual health. Seven reported notable positive associations, while four did not show sufficient effects. The review considered libido, erectile function, male infertility and testosterone, which reminds us that "sexual health" combines several different outcomes (Thu et al., 2017).

Bottom line: Tongkat Ali has a credible human signal for male libido and sexual wellbeing, but the evidence is not uniform enough to guarantee a response or replace medical treatment.

Who may be more likely to notice a benefit?

Research gives the strongest rationale where low desire overlaps with one or more of the following:

  • lower or borderline testosterone;
  • ageing-male symptoms;
  • moderate stress or fatigue;
  • reduced general wellbeing rather than a purely mechanical erection problem.

A meta-analysis of Tongkat Ali and testosterone found the clearest subgroup signal in men with hypogonadism (Leisegang et al., 2022). That does not mean raising testosterone always raises libido, but it gives a more plausible target than taking the herb when hormones, sleep, stress and desire are already normal.

How long does Tongkat Ali take to work for libido?

The most relevant trial assessed outcomes at weeks six and 12 and reported the libido change by week 12. That makes six to 12 weeks a more evidence-aligned assessment window than a promise of instant arousal. Individual experiences can appear sooner, but they are not the same as a controlled group result.

Low libido often has another cause

NHS guidance lists relationship problems, stress, anxiety, depression, erectile dysfunction, medication, alcohol, age-related hormone changes and long-term conditions such as heart disease, diabetes and underactive thyroid among possible causes of low sex drive (NHS: loss of libido).

This matters because the right response depends on the cause. If an antidepressant is affecting desire, a different herb may not solve the problem. If repeated erection difficulty reflects diabetes, blood pressure or cardiovascular health, treating it as a testosterone-supplement question can delay useful care.

Tongkat Ali is not Viagra
