This is the benefit with the strongest overall research signal. A 2022 systematic review identified nine human studies and pooled five randomised trials. Its meta-analysis found a statistically significant improvement in total testosterone, with the finding confirmed in the subgroup of men with hypogonadism. The authors still called for more research before clinical use (Leisegang et al., 2022).
One useful trial enrolled 105 men aged 50 to 70 whose total testosterone was below 300 ng/dL. Participants took 100 mg, 200 mg or placebo for 12 weeks. Total testosterone improved in both extract groups, appearing earlier and more consistently at 200 mg. Fatigue and ageing-male symptom scores also improved. However, the trial was industry-funded and did not show a significant between-group difference for free testosterone, so the result deserves interest rather than hype (Chinnappan et al., 2021).
A much smaller two-week trial in 32 healthy young men using 600 mg daily also reported higher total and free testosterone versus placebo. Its short duration and small sample mean it cannot tell us whether the effect persists or translates into meaningful long-term outcomes.

2. Stress, cortisol and fatigue
Tongkat Ali is sometimes described as an adaptogen, meaning a substance proposed to support the body's response to stress. The most frequently cited human study involved 63 moderately stressed adults who took 200 mg daily or placebo for four weeks. Compared with placebo, the extract group showed a 16% reduction in salivary cortisol and a 37% rise in salivary testosterone, alongside improvements in some mood measures (Talbott et al., 2013).
Those numbers need context. Salivary hormone measurements are not interchangeable with morning serum blood tests, the study was small, and four weeks is short. The ageing-men trial above also reported reduced fatigue scores and lower cortisol in the 200 mg group, but it used a particular standardised extract and was industry-funded.
The honest conclusion is that stress and fatigue support are plausible and supported by early human data, but not proven at the level of a clinical treatment. Tongkat Ali should not be sold as a stimulant, instant energy hit or treatment for persistent fatigue.
3. Libido and male sexual wellbeing
Tongkat Ali has a long traditional reputation as an aphrodisiac, and human research offers a signal rather than a guarantee. A 12-week placebo-controlled trial in 109 men aged 30 to 55 used 300 mg of a water extract. The authors reported improvements in the erectile-function domain, a 14% increase in sexual libido by week 12, and changes in sperm motility and semen volume (Ismail et al., 2012).
A separate six-month study in 45 men with symptoms of androgen deficiency examined 200 mg daily with or without concurrent training. The combination of extract and exercise improved erectile-function scores and total testosterone. Because exercise was part of the most successful arm, it is difficult to credit the supplement alone (Leitao et al., 2021).
Libido is affected by sleep, stress, relationships, medication, vascular health and hormone status. Tongkat Ali may be useful for some men, particularly where stress or low testosterone contributes, but it is not a treatment for erectile dysfunction or infertility. 4. Muscle strength and exercise performance
The gym claims are much stronger than the evidence. An eight-week study in 40 young men compared placebo, Tongkat Ali, resistance training and the combination. Some power measures improved in the training-containing groups, but the findings do not establish Tongkat Ali as a stand-alone strength or muscle-building supplement (Ooi et al., 2019).
There is a reasonable biological argument that correcting low testosterone or reducing fatigue could indirectly support training consistency. That is different from proving direct anabolic effects. For muscle growth, progressive resistance training, adequate protein, total energy and recovery remain far more important.
